Registration Credentials
(at the time of student registration you must bring the following with you):

Immunization Record
(including two (2) varicella vaccine)

Birth Certificate
(Must be 5 years of age by August 31, 2012)

Proof of Residency
(lease agreement, mortgage document, property tax receipt, current month's electric or gas bill - all with the 911 address and name of the parent/guardian)
